This is a collaborative virtual environment designed to promote the exchange of knowledge and best practices among individuals and organizations interested in Complex Chronic Disease Management (CCDM), at all levels, from the local to the global.

The term Complex Chronic Disease refers to patients who live with two or more diseases. These cases present a major challenge to health systems throughout the world, which tend to focus on individual conditions.

With your support it will be possible to identify, categorize and promote innovative organizations and practices to facilitate their dissemination and application worldwide.              

This Observatory is an initiative of the Andalusian Ministry of Health in Spain, through its General Directorate for Health Innovation, Systems and Technology.
