The prevalence of many chronic conditions increases with age, and seniors with multiple chronic conditions have a greater need for health care services and prescription medications than those without chronic conditions.

This study uses patient survey data from the 2008 Canadian Survey of Experiences With Primary Health Care (CSE-PHC) to understand seniors' experiences with chronic conditions and the primary health care system. The report provides important insights into the impact of chronic conditions on seniors, self-management and collaborative care in the treatment of chronic conditions and medication management.
