SeniorsThis study uses patient survey data from the 2008 Canadian Survey of Experiences With Primary Health Care (CSE-PHC) to understand seniors' experiences with chronic conditions and the primary health care system. The report provides important insights into the impact of chronic conditions on seniors, self-management and collaborative care in the treatment of chronic conditions and medication management.
