Kroniker is an observatory and online community focused on innovative practices for the management of complex diseases, with emphasis on health services research and quality optimization. The observatory is designed to support organizaciones sociales y sanitarias a mejorar sus sistemas de atención a las condiciones crónicas.Kroniker is both an Observatory and a Community of Practice focused on innovation in the field of quality of care and the management of chronic conditions. Our main objective is to help health and social care organizations transform their systems towards improved care for chronic conditions. The center is in Bilbao, Spain.
