Readiness for chronicity in Basque Integrated Healthcare Organisations: a cross sectional study

O+Berri, along with MSD and the University of Miguel Hernández de Elche has developed the ARCHO instrument (Assessment of Readiness for Chronicity in Health Care Organizations) based on the "Chronic Care Model” that allows self-assessment of healthcare organizations with respect to their degree of readiness to provide integrated care to cope with chronicity. It has been used in the existing four Basque Integrated Healthcare Organizations (IHOs) that involve primary and secondary level into one single organization.

Aims:

Know the degree of readiness to provide integrated care to cope with chronicity in Basque IHOs, using for this purpose the ARCHO tool.


Method:

Four self- assessment processes in the IHOs in 2011 and 2012 have been conducted. The assessments have been carried out by managing directors, medical directors and directors of nursing from primary and secondary care.
For each organization, the scores in the 80 interventions have been obtained, along with a list of plans and projects undertaken to support their ratings.

Results:

Individual analysis of the Basque IHOs shows not homogeneous scores in the different organizations. Nevertheless, the degree of performance in each dimension is similar in all organizations, being the "organization of the health system" one of the most valued, whereas the lowest rated dimension is "community health".

Conclusions:

The Basque IHOs are undertaking action plans in the field of continuity of care, but still with a limited deployment.
A convergence of scores in most ARCHO dimensions is expected, once these plans and projects deploy totally in the medium term.
