The continuum of care according to the classical definition is the perception of the user about the coherence of coordinated assistance, which receives over time. Freeman, posteriormente, introdujo varios conceptos operativos que deben integrarse en esta definición, como son el ámbito de la existencia identificada de responsable asistencial, de la longitudinalidad del seguimiento en el tiempo, de la flexibilidad para adaptarse a los cambios personales y sociales y finalmente de la información efectiva con el paciente, cuidadores y otros profesionales. Freeman ^3, then introduced several operational concepts to be integrated into this definition, such as the area identified the existence of responsible care, the longitudinal follow-up time, flexibility to adapt to personal and social change and finally information effectively with patients, caregivers and other professionals.
