With the aid of McKesson’s sophisticated decision support software and telephone-based services, disease management programs can be effectively delivered across large populations, including those in regional and remote areas. Their programs involve patient enrolment, assessment and GP liaison to improve illness knowledge, treatment compliance and clinical outcomes. Although face-to-face consultations with doctors and other trained health professionals are essential, telephone interactions can increase the frequency, timeliness and overall access to services. The use of clinical software for regular monitoring of each patient’s health status allows telephone case managers and other healthcare professionals to manage larger numbers of patients without the constraint of a traditional face-to-face workload, as well as to provide responsive 24/7 support and assistance.
