Marketplace of good practices in primary care

An online marketplace workshop showcasing good practices in the field of primary care took place on 01-02 June 2021. Representatives from EU Member States had the opportunity to speak directly to the originators of good practices and receive detailed information about them. The aim of the marketplace was to facilitate knowledge exchange among Member States in the field of primary care, and help Member States explore good practices in view of potential transfer and adaptation to their national context.

The experience of developing a Personalized Action Plan (PAP) in Andalusia was presented in this call, and was chosen to promote its transfer and implementation in other Member States, adapting it to the context of each country. The proposal for the joint action on the transfer of good practices in primary care, which will include this transfer, is currently under review.

Goals self-management:
Allows home care , Allows remote support to patients and their carers from health professionals , Improving support services with an emphasis on peer support, the disabled and carers , Promotes patient involvement in planning services , Refocuses the health system to support self-management
