A lo largo de la vida, las personas pueden tener necesidades de atención sanitaria o de atención social; en algunos casos, estas necesidades se presentan de forma concurrente. Especialmente en estos momentos, se hace necesaria una atención especialmente coordinada por parte de los servicios sanitarios y sociales.
Cataluña apuesta por la atención integrada social y sanitaria como herramienta estratégica para garantizar una respuesta de calidad y equitativa a las personas con necesidades de atención: personas mayores, con discapacidad, con problemática social derivada de salud mental y en general a todas las personas con necesidades complejas.
La atención integrada es una oportunidad para transformar el sistema, una palanca de cambio hacia una atención de mayor valor y con valores .
