- Improving the external validity of clinical trials: the case of multiple chronic conditions Publicado el 10/02/2014 por Diana Gosálvez Prados
-
The U.S. Department of Health and Human Services vision and strategic framework on multiple chronic conditions (MCCs) incorporates recommendations designed to facilitate research that will improve our knowledge about interventions and systems that will benefit individuals with MCCs (or multimorbidity).

Multimorbidity, the coexistence of 2 or more chronic conditions, has become prevalent among older adults as mortality rates have declined and the population has aged. We examined population-based administrative claims data indicating specific health service delivery to nearly 31 million Medicare fee-for-service beneficiaries for 15 prevalent chronic conditions.
aged, chronic disease, comorbidity, prevalence - An Answer to Chronicity in the Basque Country: Primary Care-Based Population Health Management Publicado el 13/06/2012 por Diana Gosálvez Prados
-
Chronic conditions have an impact on individuals since they represent a restraint on quality of life, functional status, and productivity of people who suffer from them but they also compromise the sustainability of health systems.

-
Long-term disorders are the main challenge facing health-care systems worldwide, but health systems are largely confi gured for individual diseases rather than multimorbidity. We examined the distribution of multimorbidity, and of comorbidity of physical and mental health disorders, in relation to age and socioeconomic deprivation.

Before approval of a new drug, the US Food and Drug Administration (FDA) requires phase 3 clinical trials demonstrating efficacy. Usually trials are conducted in homogeneous populations and rarely include individuals with multiple chronic conditions. In the United States, more than 50% of people with chronic conditions have 2 or more diseases. Twenty-eight percent of the population lives with multiple chronic conditions, including 2 of 3 older individuals. Multiple chronic conditions account for 66% of ...

The objective was to determine perceived quality of care (PQC) in patients with multiple chronic conditions, whose care is structured in Andalusia (Spain) under polypathological patients (PP) care process, and identifying aspects for its improvement.

Publicado el 06/05/2011 por Diana Gosálvez Prados

Aims and objectives.? The aim was to review available literature on research and service evaluation evidence of nurse-led case management services targeting older people with multiple chronic conditions in their own homes. This was anticipated to highlight important issues for nursing practice, healthcare policy, service provision and health service research.

-
This study aims to explore the medical needs of patients who have different combinations of multiple chronic diseases in order to improve care strategy for chronic patients. This study was based on a national probability proportional to size (PPS) sampling to older adults over 50 years old. We collaborated the files of the 2000-2001 health insurance claims and selected 8 types of common chronic diseases among seniors, for the discussion of multiple combinations of chronic ...

This paper provides the basis for rethinking the concept of “chronicity” as represented by conventional, biomedically oriented “chronic diseases” by showing how a wide range of semi-acute and acute conditions often act as if they were chronic, recurring or reappearing with periodicity in many people. Moreover, people with any given illness are more likely than people without the illness to have other unrelated illnesses, i.e., multimorbidity (more commonly referred to as “comorbidity”)
chronic illness, comorbidity, primary care - Prevalence of Multimorbidity Among Adults Seen in Family Practice Publicado el 26/10/2010 por Diana Gosálvez Prados
-
There are few valid data that describe the extent of multimorbidity in primary care patients. The purpose of this study was to estimate its prevalence in family practice patients by counting the number of chronic medical conditions and using a measure that considers the severity of these conditions, the Cumulative Illness Rating Scale (CIRS).
