En Aragón este programa establece itinerarios específicos para cada tipo de paciente, en virtud de sus características clínicas y personales. En estos itinerarios no se descarta ni la atención sanitaria más compleja, cuando así lo requiera el paciente, ni tampoco aquellos servicios específicos de cuidados y rehabilitación que deba recibir en su entorno más próximo.
- Goals Prevention:
- Promotes health and wellbeing in schools and early years , Focuses on social inequalities
- Goals Detection:
- Promotes early intervention through primary health care measures , Provides registries and patients contact systems , Advances public education on the importance of early detection
- Goals Treatment:
- Provides support health policies to facilitate the planning and integration at locally and regionally , Improving access to health services across the entire spectrum, from prevention to treatment , Ensures the best quality of health services , It facilitates the management by processes
- Goals self-management:
- Refocuses the health system to support self-management , Promotes patient involvement in planning services
