Practice description:
The CDM Care Connectivity pilot will test an integrated model of care delivery that better matches services with the needs of people with multiple chronic diseases. One of its main components is the Telecare –CareLink. A number for patients to call health professional who are available 24/7 and have electronic access to patients’ shared care plans. The Tele-Nurse advises the patient, involving the Chronic Disease Nurse, Family Physician, and Specialist based on best practice guidelines. There is also follow up support after hospital discharge, medication reminders and health counseling, and pharmacist and dietician counseling/advice.
