Delta Community Partners in Care

Delta Community Partners in Care (DCPIC) is a coalition of partners serving a rural area in the Mississippi Delta region of northwest Mississippi. This region is an underserved area for health care, where 30 percent of the population lives below poverty. Its target population is the uninsured or underinsured between the ages of 21 and 64 who have a diagnosis of diabetes, hypertension, or both. DCPIC attempts to reduce the barriers affecting its target population by providing outreach case management services. Services offered DCPIC uses a community-based case management model to improve the health status and risk factors in its target population. Caseworks are trained social workers, nurses, and lay health workers who work directly with patients who have a diagnosis of diabetes, hypertension, or both. The caseworks use a holistic approach and include support groups and education.

Promotes early intervention through primary health care measures , Provides registries and patients contact systems , Shows new evidence on the importance of early detection, including guidelines of effective practices , Advances public education on the importance of early detection
Goals Treatment:
Provides support health policies to facilitate the planning and integration at locally and regionally , Strengthens cooperative local groups to provide integral services , Promotes the training of health professionals , It facilitates the management by processes , Emphasizes the care of pluri-pathological and fragile patients
Goals self-management:
Refocuses the health system to support self-management , Promotes patient involvement in planning services , Improving support services with an emphasis on peer support, the disabled and carers
