The object of this project is to develop a home telemonitoring system for people with cardiac failure or requiring palliative care. This telemedicine service try to ease the remote communication between patients and proffessionals and monitore the health status of patients (physiologic data such use blood pressure, weight, electrocardiogram, oxigen saturation) through scheduled phone contacts or the web. The economical and health care impact will be analyzed, as well as satisfaction of patients and health care professionals. This service will be fully integrated in the assistencial services of the institution. Furthermore, new chronic conditions and telemonitoring devices may be included in future.

Telemonitoring is remote monitoring of physiological parameters of patiens. It represents a tool of telemedicine.
In heart failure, control of weight and adjustment of the medication doses when weight gain happens (due to decompensation) has been shown to be an effective measure for hospital admission prevention.
