This is a program designed to help people living with chronic diseases understand their conditions and how to take better care of themselves. The Office of Vermont Health Access (Medicaid, Dr. Dynasaur, VHAP, or PCPlus) has teamed up with APS Healthcare to offer the program, which is free to eligible invidivuals. The Chronic Care Management Program offers a nurse to provide education on overall health issues and ways to improve levels of quality of life. Nurses can help patients to talk to physicians about treatment issues. They also work with a team of social workers as well as care coordinators and health educators to assist patients with any other of their health needs.
