Taddle Creek Family Health Team/Sunneybrook Hospital Family Practice

Taddle Creek Family Health Team,  Toronto, Ontario,  is a primary care facility made up of family physicians, nurses, nurse practitioners, pharmacists, social workers and dieticians. This staff complement along with our consulting internist and psychiatrist have been offering a consultation clinic with complex patients and their families present to offer a multi-faceted, real time patient centered assessment and treatment plan.

The program has been extended to 3 other family health teams as well as via telemedicine to solo community-based family physicians who access the team-based service for a patient in their office.
