Structure and communication, basic for chronic patients

The chronic patient may be a bigger player in managing their own illness, although this requires tools that allow them to prepare and share responsibility. The best way to address this growing group of patients is providing those tools-to information as basic weapon, and approaching from the system as comprehensive as possible.
The approach to these patients should be more comprehensive and humane.
