Angel Lee and Ong Yew Jin wrote the first draft of this chapter in English. Alejandro Jadad approved it before it was translated into Spanish. The English and Spanish versions were uploaded onto the OPIMEC platform, where they received important contributions from Maria Nabal (in Spanish), Scott A Murray (in English) and Jackie Bender (in English). Alejandro Jadad incorporated such contributions into a revised version of the chapter, which was approved by Angel Lee and Ong Yew Jin, and the other contributors.
