San Juan de Dios del Aljarafe Hospital, Sevilla, has developed a Health Service Model centered in the patient, which facilitates collaboration between the different care levels, obtaining a high degree of professional satisfaction. In this article the model y partially described, showing its main components, and some parameters used for evaluation of results, and reviewing the models of continuity of care previously reported in the literature.
