21st Century Challenge for Healthy Aging: Balancing Living Well with the Reality of Multiple Chronic Conditions

To raise awareness of the challenges and opportunities for healthy aging for older adults in the U.S., the White House Conference on Aging will present the first in a series of webinars on this important topic in December. 21st Century Challenge for Healthy Aging: Balancing Uving Well with the Reality of Multiple Chronic Conditions is being hosted in collaboration with the Office of the Assistant Secretary for Health, U.S. Department of Health and Human Services (HHS), and will be held on Thursday, December 11, 2014, 4:00 p.m.- 5:00 p.m. EDT.

HHS leads the Strategic Framework on Multiple Chronic Conditions, a roadmap for improving the health status of persons with multiple chronic conditions. Since the Framework's release in December 2010, HHS agencies and external partners have worked together to align their respective programs, activities, and initiatives to advance the Framework's vision of optimizing health and quality of life for persons with multiple chronic conditions.
